Referrals to the Small Animal Hospital, strongly preferred
To make a referral to a service in the SAH, please fill out the referral form to provide us information and email the patient records to referral@vetmed.wisc.edu. If it is an emergency, please call 608-263-7600.
Referrals to the Large Animal Hospital
You may call 608-263-6300 Monday-Friday, 8 am-5 pm, to reach the Large Animal Reception Desk staff. If you want to make a patient referral or have questions, the receptionist will forward your call to the clinician on the medicine or surgery service as appropriate. After 5:00 pm and all during the weekends, the emergency service calls to the Large Animal Hospital are forwarded to an answering service. This service will immediately contact the clinician on emergency call duty, and you will receive a call back shortly thereafter.
Client Medical Records
So that we may deliver the best possible care to the patient you are referring, please provide us with the relevant medical history and lab results. You may send this information with your client for their appointment at UW Veterinary Care, or you may fax it to 608-265-8276 or email referral@vetmed.wisc.edu your records prior to the appointment.
ER Status
ER Status and Hospitalization capacity change frequently. We highly recommend calling ahead to verify availability. We can be reached at 608-263-7600
Due to staffing shortages and to ensure proper care of the hospitalized patients, UWVC Small Animal Emergency Care closes as the need arises and we highly recommend you to call us ahead of time. In the event, our ER service is closed, please seek medical help at another emergency clinic.
