Create your account

  1. Visit the UWVC Client Portal
  2. Click Sign up…
  3. Enter your First Name, Last Name, and Email Address that we have on file for you. If you are uncertain, please call us at 608-2623-7600 to verify your information.
  4. You will get this notification
  5. Check your email and click the Account Activation Link
  6. Create your username and password > Confirm
  7. You will arrive at a login page. RE-TYPE the username and password you just created > Log In
  8. Click on your pet’s name to view your pet’s medical record. Currently, finalized discharge instructions, patient updates, appointments, and prescriptions display.
  9. Click “Update Photo” to upload a photo of your pet, click “view” (eye icon) for Discharge instructions, medical records by visit
  10. If you have trouble registering, please call 608-263-7600. We can verify the name and email address that we have on file for you.
